Our Mission Statement:

We are a non-profit organization whose mission is to provide quality healthcare to people of all ages with financial need. CHC has been serving Decatur County since 1984.

We are funded by Decatur County United Fund, Decatur County Community Foundation, and outside donations. We are staffed by volunteer doctors, dentists, nurse practitioners, registered nurses, and dental hygienists.

Our physicians are available to conduct physical examinations. They treat minor illnesses such as the flu or common colds as well as offer proper education if you suffer from chronic dieses such as high blood pressure, diabetes, emphysema, etc.

We are proud to offer some Dental Services for all ages, right here at our clinic. Our Dentists and Dental Hygienists will provide general dental procedures as well as patient education.

When it comes to your lab services, x-rays and medications we know the costs sometimes become quite expensive. We will gladly provide you with the necessary information and forms on how you may be able to receive assistance with the costs of your medications and labs for you and your family.

Our clinic is always in need of medical and dental providers and services.
